{"id":15342,"date":"2014-07-09T16:40:04","date_gmt":"2014-07-09T20:40:04","guid":{"rendered":"https:\/\/www.saratoga.com\/saratogabusinessjournal\/2014\/07\/aarp-states-improving-long-term-care-but-there-is-still-room-for-improvement.html"},"modified":"2014-07-09T16:40:04","modified_gmt":"2014-07-09T20:40:04","slug":"aarp-states-improving-long-term-care-but-there-is-still-room-for-improvement","status":"publish","type":"post","link":"https:\/\/www.saratoga.com\/saratogabusinessjournal\/2014\/07\/aarp-states-improving-long-term-care-but-there-is-still-room-for-improvement\/","title":{"rendered":"AARP: States Improving Long-Term Care, But There Is Still Room For Improvement"},"content":{"rendered":"<p> While states are making measureable progress<br \/>\nin improving long-term services and supports<br \/>\n(LTSS) &#8211; which includes home care services,<br \/>\nfamily caregiver supports, and residential<br \/>\nservices such as nursing homes &#8211; widespread<br \/>\ndisparities still exist across the country, with<br \/>\neven top performing states requiring improvement,<br \/>\naccording to the AARP.<\/p>\n<p>Further, the pace of change remains slow,<br \/>\nthreatening states&#8217; ability to meet the needs of<br \/>\nthe aging population, the study said.<\/p>\n<p>Raising Expectations: A State Scorecard on<br \/>\nLong-Term Services and Supports for Older<br \/>\nAdults, People with Physical Disabilities and<br \/>\nFamily Caregivers, evaluates 26 indicators in<br \/>\nfive key dimensions that make up the LTSS<br \/>\nsystem in each state. The state-by-state report<br \/>\nupdates the inaugural 2011 LTSS Scorecard,<br \/>\naccording to AARP.<\/p>\n<p>The highest ranked states &#8211; Minnesota,<br \/>\nWashington, Oregon, Colorado &#8211; can offer lessons<br \/>\nfor possible solutions, but they too have<br \/>\nopportunities for improvement, the study said.<br \/>\nLeading states have implemented laws and<br \/>\npolicies that build stronger Medicaid programs<br \/>\nand support family caregivers.<\/p>\n<p>These laws include paid sick leave, nurse<br \/>\ndelegation of health maintenance tasks, and<br \/>\ndevoting more Medicaid dollars to home and<br \/>\ncommunity based services. Top states also<br \/>\nhave lower use of nursing homes and minimize<br \/>\ndisruptive transitions between care settings,<br \/>\nproviding lessons for lower-ranked states.<\/p>\n<p>&#8220;Americans want to live independently in<br \/>\ntheir homes and communities as they age,&#8221; said<br \/>\nAARP Senior Vice President for Public Policy<br \/>\nSusan Reinhard. &#8220;States are starting to step up<br \/>\nto meet this challenge, but more must be done<br \/>\nin a short time to meet the changing needs of<br \/>\na growing older population.<\/p>\n<p>&#8220;By addressing the findings in the Scorecard,<br \/>\nstates have the power to affect positive change<br \/>\nboth immediately and in the long run so older<br \/>\nresidents and their family caregivers can access<br \/>\nthe quality long-term services and supports<br \/>\nthey require.&#8221;<\/p>\n<p>The LTSS Scorecard evaluates performance<br \/>\nin five key dimensions: affordability and access,<br \/>\nchoice of setting and provider, quality of life<br \/>\nand quality of care, support for family caregivers,<br \/>\nand effective transitions.<\/p>\n<p>Officials said new indicators this year<br \/>\ninclude length of stay in nursing homes and<br \/>\nuse of anti-psychotic drugs by nursing homes,<br \/>\nraising serious concerns about the quality of<br \/>\ninstitutionalized care.<\/p>\n<p>Even facing tight budgets following the<br \/>\nGreat Recession, all states made progress in<br \/>\nat least one of the Scorecard&#8217;s 26 indicators.<\/p>\n<p>According to the study, more than half of the<br \/>\nstates (29) improved their laws and supports<br \/>\nfor family caregivers &#8211; including expanding<br \/>\nfamily and medical leave requirements and<br \/>\nlaws requiring sick days, and allowing nurses<br \/>\nto delegate health maintenance tasks to home<br \/>\ncare workers &#8211; and, 28 states improved the<br \/>\nfunctioning of Aging and Disability Resource<br \/>\nCenters, which help residents navigate available<br \/>\nservices and supports. At the same time,<br \/>\nthe cost of LTSS remains unaffordable for<br \/>\nmiddle income families.<\/p>\n<p>The single strongest predictor of overall<br \/>\nLTSS system performance is the reach of a<br \/>\nstate&#8217;s Medicaid LTSS safety net. Nearly half<br \/>\nthe states (24) increased the percentage of Medicaid LTSS dollars that support home and<br \/>\ncommunity-based services &#8211; the care setting<br \/>\nthat most Americans prefer. However, widespread<br \/>\ndisparity exists across the states on this<br \/>\nimportant indicator. While the five top-ranked<br \/>\nstates dedicate 62.5 percent of Medicaid LTSS<br \/>\ndollars to HCBS, the lowest-ranking five only<br \/>\ndevote 16.7 percent.<\/p>\n<p>&#8220;The report underscores the importance of<br \/>\npublic policies, including those that support<br \/>\nproviding care for people in their own homes<br \/>\nand communities,&#8221; said Melinda K. Abrams,<br \/>\nvice president for health care delivery system<br \/>\nreform at The Commonwealth Fund. &#8220;Without<br \/>\nstrengthening such services, we put millions of<br \/>\nfrail elderly and disabled at risk for frequent<br \/>\nemergency room visits, hospital stays or poor<br \/>\nquality of care.&#8221;<\/p>\n<p>Further, states with more effective transitions<br \/>\n&#8211; both those that help people move out of<br \/>\ninstitutions and back to the community as well<br \/>\nas those between care settings &#8211; performed<br \/>\nbetter overall in the Scorecard.<\/p>\n<p>The Scorecard&#8217;s authors warn of the demographic<br \/>\nimperative to hasten progress. &#8220;In just<br \/>\n12 years, the leading edge of the Baby Boom<br \/>\nGeneration will enter its 80s, placing new<br \/>\ndemands on the LTSS system.
