A Clifton Park physician and his Rensselaer
\nCounty-based partner have created a health care
\nmodel, based on affordability and convenience,
\nwhich they say may be the first of its kind in the
\nnation.<\/p>\n

Dr. Michael Bibighaus and Keith Algozzine, a
\nphysician assistant from Averill Park, formed Upstate
\nConcierge Medicine about a year ago, but went
\nlive in September following several months of beta
\ntesting. Both men work out of their homes, although
\nthe business uses a Clifton Park mailing address.<\/p>\n

The model combines telemedicine, through
\nwhich patients receive medical assistance over the
\nphone or computer, with concierge healthcare, in
\nwhich the patient pays a monthly or annual retainer
\nto see a physician as often as they wish.<\/p>\n

“So the phrase that I coined is concierge telemedicine,”
\nsaid Algozzine, who is CEO of Upstate
\nConcierge. “No one is doing this across the country
\nthat we’re aware of, so we’re the first. But we believe
\nthis is actually the right way to treat patients.”<\/p>\n

Bibighaus, president and chief medical officer of
\nUpstate, and Algozzine were part of the team that
\nfounded one of the country’s largest telemedicine
\ncompanies, and still are part owners of that business.
\nAlgozzine said that while the national “dial-a-doc”
\nmodel has its place, he and Bibighaus wanted to
\ncreate something that was more local and personal.<\/p>\n

With Upstate Medical, patients pay a monthly
\nfee of $12 for an individual and $14 for a family to
\nobtain access to healthcare professionals 24 hours
\na day, 7 days a week. Most interactions take place
\nover the phone, although videoconferencing, e-mail
\nand text messaging–sending photos of a rash, for
\nexample–are also available.<\/p>\n

The idea is to prevent someone with a relatively
\nsimple, and easily diagnosed, complaint from having
\nto take time away from their job or family to go to
\na physician’s office. If necessary, prescriptions are
\nsent to a local pharmacy for pick-up, or the patient
\nis referred to a specialist, imaging center or lab.<\/p>\n

In some cases, Algozzine or Bibighaus will make
\na house call, usually as a follow-up.
\n“For example we had a patient with a bad leg
\ninfection,” Algozzine said. “He was local, so we
\ntold him we would stop by his house in two days to
\ncheck on him.”<\/p>\n

At the very least, any patient with an acute
\nproblem gets an automatic call back within three
\ndays to ensure treatment is working. Clients also
\nare given their own patient portal, where all their
\nmedical records are housed electronically.<\/p>\n

Algozzine said the monthly retainer was kept low
\nto make Upstate Concierge accessible to as many
\npeople as possible.
\n“We want the grocery bagger who doesn’t have insurance to be able to afford it, as well as the rich
\nman or woman living on the hill,” he said.<\/p>\n

The practice also has a model for businesses that
\nwant to sign up their employees, on the theory that
\nconcierge telemedicine saves time, reduces absenteeism
\nand may lower insurance costs by avoiding
\nthe claims that go with office visits. The price is
\n$160 a year per employee, and Algozzine said several
\nbusinesses, of all sizes, have expressed an interest.<\/p>\n

Upstate Concierge went live with a few hundred
\npatients, but Algozzine said the company hopes to
\nbuild to several thousand clients. It is accepting
\nmemberships from anywhere in the state, but is
\nfocusing on an upstate area Algozzine defined as
\nrunning from about Glens Falls down to Columbia
\nCounty, and west to about Amsterdam.<\/p>\n

“That is where we have the most local expertise,
\nand can do a home visit if we need to,” he said.
\n“Where I can tell you the best cardiologist to go to
\nif you need it, where I can send you to this imaging
\ncenter if you need an x-ray, or to this lab if you need
\na test. Really the highest level of service we can give
\nis in that region.”<\/p>\n

At the moment, the Upstate Concierge team
\nconsists of Algozzine and Bibighaus, who together
\ncover about 90 percent of calls, as well as a few
\nother providers and support staff. Most technology
\nis outsourced, and an answering service screens
\nall calls to be sure the person is a paying member.
\n“We are going to keep it small,” Algozzine said.<\/p>\n

“You’d be amazed. With a few hundred members we
\nonly get one or two calls a day, even on a 24-hour
\nbasis. So it is not overly difficult for one or two people
\nto handle it. When you move up to 10,000 members,
\nlike in our previous [national] model, even then you
\nget maybe 15 calls a day. So even when we get up to
\nbigger numbers, we will be fine with five, six, maybe
\nseven doctors taking turns on call.”<\/p>\n

Both telemedicine and concierge care are growing
\nnationally, in large part, because of a shortage of
\nprimary care doctors. This situation was made worse
\nby the Affordable Care Act, or “Obamacare,” which
\nincreased the pressure on physicians by adding more
\npeople to the insurance rolls.<\/p>\n

“There are not enough primary care doctors to
\nadequately take care of all of the people that need it
\nto the level that they need it,” Algozzine said. “Even
\nif you have a pretty good primary care doctor, he or
\nshe is only open four or five days a week for limited
\nhours. So there are automatically limitations with
\na brick and mortar building. We are able to provide
\nan immediate access point for these quick and easy
\nproblems.”<\/p>\n

That does not mean brick-and-mortar is out of
\nthe question for Upstate Concierge. Algozzine said
\noffices are a possibility in the future, depending on
